Below is the majority of the paperwork
that we use
here at our office. All new patients (including auto
accidents,
worker's compensation, or health insurance) must fill out the New Patient Paperwork
packet. Cash patients need to fill out the same packet, but
do not need
the "Assignment of Health Benifits" form. If your current injury was caused by a work related injury (worker's compensation claim) please fill out the Worker's Compensation form. If it is realated to an auto accident (personal injury), please fill out the Accident Paperwork form. |
